Crimson Mischief is looted from Orin, after finding the Murder Tribunal and acquiring the Amulet of Bhaal, which opens the Bhaal Temple (X: 61 Y: 1004). You can reach this point by following the steps in the Get Orin’s Netherstone main quest during Act 3.
