Mythic Items in ESO are one-piece set items that give unique and powerful stat bonuses for a specific gear slot. Currently, Mythic items exist for the Helmets, Shoulders, Chest, Legs, Gloves, Belt, Feet, Necklace and Ring slots. There are no Mythic Weapons available in ESO as of yet. Also keep in mind that while you can collect as many Mythic Items as you want, only one Mythic set piece can be worn at a time.

How to get Mythic Items in ESO
To start getting Mythic items, you must first unlock the Scrying and Excavation Skill Lines as part of the Antiquities System in ESO. These skills will let you locate and excavate Mythic Leads which can then be combined to create a Mythic Item. Scrying and Excavation are unlocked as part of the Greymoor Chapter, so this DLC must be purchased to participate in Antiquities.
Once you have purchased the Greymoor Chapter, head to the main city of Solitude in the zone of Western Skyrim and locate the Antiquarian Circle. Here you can learn the basics of the Antiquities System as well as unlock the Scrying and Excavation skill lines.
When you first start with the Antiquities System you will only be able to find and excavate basic Treasures and Furnishings. However, once you reach Level 7 in the Scrying Skill, Mythic Items will finally become available and you can begin collecting the pieces which you can then turn into Mythics.
Each Mythic item has 5 pieces or fragments you must collect in order to create it. These pieces are often not a 100% drop rate so some Mythic Items can take quite a while to complete.

How Mythic Leads Work in ESO
Leads are clues that allow you to start looking for Antiquities (and Mythics!) in a specific zone. In this case, we are looking for Leads for Mythic items. Upon finding a new Lead, it will be placed into the Antiquities Codex section in your journal, allowing you to Scry for its location and then dig it up with Excavation. Most Mythic Items in ESO require 5 Leads to be Scryed for an Excavated. Combining all 5 Mythic Leads will place the Mythic Item into your inventory.
Within your Antiquities Codex you can navigate to different Leads and Mythics by looking at a specific zone or searching all recently discovered leads, and you can pick which lead you want to scry first and sort them by their location, rewards, or difficulty. You will also need to be in the zone where the excavation site will be, otherwise you cannot scry for that Lead.

How do I get Mythic Leads?
When you join the Antiquarians Circle in Solitude, you’ll unlock many basic Leads automatically for antiquities all over Tamriel. However, most Mythic Leads are more difficult to find. Yes, you will find some seemingly at random, but typically a Mythic Lead will have a very specific drop instance or location and this will be the ONLY way to obtain it. For example, The Larimar Gems Lead for the Oakensoul Ring can be found by killing the Titanclaw World Boss located in the zone of Stormhaven.
How to use Leads
Once you have a Lead for the Antiquities System the work really begins! Review the text of the Lead in your Antiquities Codex to see which zone contains the Antiquity or Mythic fragment, then travel to that zone and use your Scrying Skill to reveal possible dig site locations. Once you have located an antiquity dig site, you can then begin excavating it to obtain the Antiquity or Mythic Lead you were looking for. Remember, Mythic Items require FIVE Mythic Leads to be both Scryed and Excavated so each Mythic can be time-consuming to unlock but most are well worth it!
